Importance of Care Coordination
EasCare strongly believes in developing customized, client-centered programs that facilitate improved coordination, collaboration and integration between health programs and services. Effective coordination of existing resources through multidisciplinary teams has been a highly recognized approach in improving the provision of timely and seamless access to primary care services, thus closing the loop on patients’ needs as they navigate the health care system. We have invested in innovative technology that can be used to enhance the collaboration among health care providers and facilitate care coordination for the patient.
Communication and Collaboration
Communication and sharing of information among service points is a critical factor in patient outcomes. The lack of interaction and collaboration between health care service providers can result in redundancies and inefficiencies in service delivery. The absence of information sharing across the health care continuum results in the inability to track patients throughout their journey across the health care system and hinders the coordination of patient care. Eascare has developed a custom application and interface that allows us to push and pull patient information directly from our client's medical records system into our electronic patient care record. We are the only medical transportation provider in Massachusetts that is in the process of becoming state certified to share patient medical records across the state health information exchange.
